How To Become A Patient

If you are a new patient to the community, who has not been assigned a Family Physician at the Marathon Family Health Team, and has not been enrolled to the Marathon Family Health Team, you must register with Health Care Connect first.

Once you are registered with Health Care Connect, you will be attached to a Family Physician or Nurse Practitioner with the Marathon Family Health Team. At your first visit to the clinic, you will be asked to complete our New Patient Questionnaire. Please provide as much information as you can on this form as it will assist us to provide you with the best health care possible.

Health Care Connect: Connecting people to Care

Health Care Connect is a program that will help people without a regular health care provider find one in their community.

The program identifies doctors or nurse practitioners who are accepting patients and links them with people who are in need of a family health care provider.

Registering for Health Care Connect is easy.

When you call 1-800-445-1822 (Monday through Friday, 9 am to 5 pm) you will speak to someone who will ask you a short series of questions and sign you up for the program. You can also sign up online at:

Priority is given to those individuals with greater health needs.

You will be referred to a family doctor or nurse practitioner here at the Marathon Family Health Team who is accepting new patients.

You will need to have your OHIP number ready.

The Marathon Family Health Team is accepting new patients from the Marathon, Pic River First Nations and Pic Mobert First Nations areas, for placement in the practices of:

  • Dr. Sayali Tadwalkar
  • Dr. Ryan Patchett-Marble

We work exclusively with Health Care Connect. You may advise Health Care Connect that the Marathon Family Health Team is your Clinic of choice.
