Prepare For Your Visit

In an effort to help you maximize your clinic visit, we encourage you to review the following for EVERY visit:

  1. Prioritize: Prepare a list of things that you would like to discuss with the physician at the visit. Highlight the 3 most important ones for discussion as you may not be able to address all of the issues on your list. Another appointment can be made to discuss those issues that remain.
  2. Keep a diary: In relation to the health care issues on your list, keep a diary of what your symptoms were, what made them start, and what made them go away. Keep track of any associated symptoms like fever, nausea, diarrhea, pain and weight loss. Try and record what you have tried at home for the problem (stretching, heat, medications). It is also helpful if you can indicate whether you have seen any other physicians for the same problem and whether you had any investigations or tests (x-ray, ultrasound, blood work).
  3. Bring your medications: If you take any medication regularly, including prescription, over the counter medications and complementary medicines, please bring them with you to the appointment. This is helpful for your physician to know, even if you don’t have questions or concerns about what you are taking.
  4. Forms: If you have forms that you know you would like completed after the visit, please be sure to fill in your portion before you come so that delays in completing the forms are minimized.

As you arrive for your visit:

  1. Arrive early: Try to arrive about 10 – 15 minutes early, so that the administrative staff can greet you and answer any questions that you may have about the healthcare service you are about to receive.
  2. Bring your health card: Please have your Health Card available for scanning by the Front Desk Receptionist as you arrive to the office.
  3. If you are delayed: If you are running behind in your schedule, but still expect to arrive for a portion of your appointment, call ahead and advise the Administrative staff that you will be a few minutes late. Every effort will be made to accommodate your full appointment time, but there is the possibility that if you arrive late, your appointment may need to be shortened.
  4. Enrolling with MFHT: You will be asked to complete a Patient Enrolment Form if you are interested in enrollment with the practice.
  5. Be prepared to settle your outstanding account if you have a balance owing. We accept Debit, Visa or Cash.
