Enrolment Information
Patients of the Marathon Family Health Team will now be asked to enroll to the group.
Benefits of enrollment include:
- Enhanced reminder services for wellness and illness prevention strategies such as:
- Pap tests and colorectal screening
- Flu shot
- Infant immunization
- Mammograms
- Smoking Cessation
- Management of patients with chronic illnesses such as diabetes
- Other screening tests
- Participation in extended hours on evenings and weekends (when applicable).
- Access to primary care treatment, emergency treatment or telephone health advice 24 hours a day, 7 days a week.
- Alternate appointments with other healthcare providers of the Family Health Team, such as Registered Nurses, Social Worker, Registered Dietitian, Nurse Practitioner, Healthcare Assistant and part-time specialty programs such as Cardiology and Psychiatry.
When you sign the enrolment form, you will be consenting to release personal information about yourself to the Ministry of Health. Please make sure you understand the following before you sign the form:
You are agreeing to allow the Ministry of Health to access our billing information regarding healthcare services given to you for preventive health services such as annual health exams, mammograms, immunization and colorectal screening.
This information is combined with the same information gathered from other medical clinics in the Province of Ontario, and at NO TIME are you personally identified.
If at any time you feel uncomfortable with the Ministry of Health having access to this specific information, you may withdraw your enrolment to the family health team.
You may withdraw from enrolment at the Marathon Family Health Team at any time. Please ask our staff to assist you with this process, should you change your mind about being enrolled with our clinic.
