The Marathon Family Health Team was thrilled to present at the Association of Family Health Teams of Ontario (AFHTO) Conference on October 24-25, 2024, in Toronto!
Our Lifestyle Medicine Program helps patients make healthier choices by focusing on six essential pillars: nutrition, physical activity, sleep, stress management, avoiding risky substances, and building social connections—all supported by a qualified coach.
The program aims to prevent and manage chronic conditions like heart disease and diabetes. During the 8-month pilot, participants saw encouraging results, with their health rating increasing from 5.9/10 to 7.5/10 and greater confidence in making healthy changes. Flexible scheduling, group sessions, and one-on-one coaching have made the program accessible and effective for many.
As healthcare needs in our community continue to grow, this proactive approach empowers patients to build lasting, healthy habits for a better quality of life. We’re excited to have shared our progress and results with other teams committed to innovative primary care!
